Provide as much as you reasonably can. Missing information should not prevent you from making an initial report.

  • Your name and a phone number or email for follow-up.
  • The affected person's age or initials and relevant health information.
  • The complete product name, form or strength, and amount used.
  • The lot or batch number, expiration date, and UID, if present.
  • When use and symptoms began, what happened, and the current outcome.
  • Medical care received and other medicines or supplements used at the time.
  • Where and when the product was purchased.
  • The container and product label, kept available for reference.
